TO PERSONAL DATA TREATMENT
I, undersigned, having received the information by the
owner or delegated authority on privacy regulations
according to the art. 13 of the Italian Law 196/2003,
am aware that the treatment of my data will regard sensitive
data, likely to reveal racial and ethnic origins, religious,
philosophical and other kinds of beliefs, political
opinions, membership of parties, trade unions, religious,
philosophical, political or trade union associations
or organizations, health conditions and sexual habits
and I express my consent to the treatment of my personal
data for the ends, and within the limits of, the nature
of the above-mentioned information.
According to the paragraphs 1341 and 1342 of the civil
code, I, undersigned, agree with the following articles
of the Student Guidelines: Art.11, 12, 13, 14 and 15
of Section III (Lates and Absences), Art. 16, 17, 18
(Grading System) and Art.29 (Reimbursement of Fees to
Withdrawing Students)

I declare to be
fully aware that attendance at Polimoda courses are
mandatory and that exams must be taken at the pre-established
times (there are no make-ups). Polimoda reserves the
right to change the course of studies for the courses
offered at Polimoda without forewarning. I understand
that is my duty and not that of Polimoda
the obtaining of student visas and the applying
for permits of stay required by the
Italian legislation, before the beginning of my attendance
in the course.
Polimoda will provide
you with all the instructions in order to ensure your
successful application for the above mentioned documents.
Since you can apply for a student visa only in your
home country, make sure you have followed the right
procedure and obtained the necessary papers before
leaving your country. |
